The scalp: the thin graft by 1st intention in emergency surgery. Apropos of 4 cases

The authors report four cases of scalp grafts and review the classical concepts concerning the mechanisms of the lesions. The therapeutic approach in each case consisted of a thin dermo-epidermal graft, which was necessary because of the incorrectly positioned and necrotic scalp, unusable scalp and loss of scalp tissue. However, the development of microsurgery offers a different possibility: emergency reimplantation. The requirements and advantages of this technique are recalled. Overall, reimplantation should be chosen whenever the loss of substances exceeds 1/3 of the total area of the scalp. The thin graft, taken from the avulsed tissue is a very valuable approach when reimplantation is impossible or hazardous.
