Sphincter resistance and stress incontinence

We present herein 71 cases of stress incontinence; the exploration has been focused on the competence of the bladder neck and on the continuous maximal urethral pressure. The bladder neck is incompetent in 15% of the cases in the prone position, and in 84% in the supine position. The closure pressure is less than 35 cm H2O in 30%. Moreover we described a stress urethral dysreflexia in another 30%; it consists of an urethral relaxation (sometimes confirmed by thhe EMG) of more than 20 cm H2O and appearing after coughing. These observations lent us to modify our technique of colposuspension, in some cases.
