The current status of alveolar ridge augmentation using bone grafts is presented for the surgical management of the atrophic mandible. The procedures presented are the alveolar onlay bone graft with a case report, 3 modifications of the visor osteotomy, the reverse-visor osteotomy, the sandwich osteotomy, 2 variations of the combined visor-sandwich osteotomy and hydroxylapatite/cancellous bone augmentation. Improved alveolar ridge contour, enhanced masticatory function and increased denture stability are the advantages discussed. Anesthesia and paresthesia, uncomfortable rib or iliac crest donor sites and inevitable alveolar ridge resorption are the disadvantages discussed.
