The transmission of the human immunodeficiency virus by breast feeding remains highly controversial, resulting in the issue of guidelines advocating and discouraging breast feeding by seropositive mothers. It is proposed that individual evaluations are made and that only specific cases, such as seropositive mothers in an advanced stage of acquired immune deficiency syndrome or mothers identified as having received a seropositive transfusion, be discouraged from breast feeding. This policy would be helpful, particularly in developing countries, enabling the benefits of breast feeding to be maintained.
