Effects of various drugs and fluid on the blood coagulation- fibrinolysis and kinin systems in canine hemorrhagic shock

Effects of various drugs which might have antishock property and fluid on blood coagulation-fibrinolysis and kinin systems were investigated during hemorrhagic shock in dogs. Mongrel dogs (48) were divided randomly into 5 groups: control group treated with lactated-Ringer's solution alone (8 dogs); dexamethasone group treated with dexamethasone 5 mg/kg (10 dogs); aprotinin group treated with aprotinin 10,000 U/kg per 60 min, (10 dogs); MR-20 group treated with MR-20 50,000 U/kg per 60 min. (10 dogs); and coenzyme Q10 group treated with coenzyme Q10 10 mg/kg per 60 min. (10 dogs). Each dog was anesthetized with i.v. pentobarbital 30 mg/kg, followed by tracheal intubation and ventilated artificially with room air. After obtaining the control blood sample, .apprx. 40 mg/kg blood was withdrawn in 30 min and the mean arterial blood pressure was reduced to .apprx. 50 mmHg. Thirty min after the start of the state of hemorrhagic shock, the above mentioned drugs were administered i.v., which was followed by infusion of lactated Ringer's solution 20 ml/kg over 30 min. Hematological measurements were made on 5 occasions during the procedure. These included platelet counts, prothrombin time, activated partial thromboplastin time, plasma fibrinogen levels, plasma plasminogen levels, plasma antithrombin-III activity, plasma .alpha.2 plasmin inhibitor activity and plasma kallikrein like activity. Administration of fluid alone induced only a state of consumption coagulopathy and hyperfibrinolysis following hemorrhagic shock. Administration of dexamethasone or MR-20 (a urinary tripsin inhibitor) improved the blood coagulation system and suppressed the fibrinolysis system slightly. Aprotinin showed anticoagulant action and remarkable antifibrinolytic action. Coenzyme Q10 had no prominent influence on the blood coagulation system, but suppressed the fibrinolysis system. Dexamethasone, aprotinin, MR-20 and coenzyme Q10 appear to be effective for the treatment of hemorrhagic shock in dogs from the point of view of blood coagulation-fibrinolysis and kinin systems.
