The indication for the extraction of teeth is not only essentially influenced by the position of the dentition in the sagittal outline form of the face, and by the direction and intensity of the jaw growth. The irregularity of the dental arch resulting from the differences of the broad widths of the teeth and the space available must be considered the most essential factor. The position of teeth in the face is decisive for stating whether the crowding can be equalized by anterior shifting of the dental arch, or not. By calculating the overall irregularity a limit was set up to point out where the lower incisors can be repositioned without extractions. The type of malocclusions, especially the jaw growth to be expected may decisively modify the therapeutic intent. The condition between the upper and lower dental arch and their symmetry represent the occlusion factor. Only after having taken the decision on extractions and on the method of treatment, the type of appliance to be used is determined in consideration of its effectiveness.
