Exercise echocardiographic evaluation of postoperative cardiac function in patients undergoing open mitral commissurotomy

The cardiac reponse to exercise in patients undergoing open mitral commissurotomy (OMC) is not fully understood. In this study, left ventricular function was measured at rest and during supine exercise by bicycle ergometer in 7 normal subjects and 17 patients after OMC. Left ventricular performance was assessed by M-mode and two dimensional echocardiography. The results were summarized as follows: (1) In normal subjects, the cardiac output increased significantly with increases of heart rate and stroke volume. The increase of stroke volume was resulted in increases of ejection fraction, mean VCF, end-diastolic volume. (2) In patients after OMC, the increase of cardiac output was produced by the increase of heart rate. The stroke volume was not increased because of the decrease of end-diastolic volume and no change of mean VCF. (3) Cardiac response to exercise in patients with Sellors' classification type I seemed to be almost normal. In patients with type III, cardiac response was poor because of the decrease of end-diastolic volume and no changes of mean VCF and cardiac output. In patients with type II, cardiac output and mean VCF increased, but end-diastolic volume decreased.
