Long-term prognosis of convulsive disorders in the first year of life: febrile and afebrile seizures

A follow-up study was made on 166 cases with febrile and afebrile seizures in the 1st yr of life. All patients were followed until 6 yr of age or older. Subjects were divided into 4 small groups: groups A, febrile convulsions only; group B, febrile convulsions, evolving afebrile seizures; group C, afebrile seizures with normal mental and physical development and seizures stopped before the age of 3; group D, afebrile seizures other than group C. Comparison among these 4 groups were made with regard to etiologic factors, clinical features. EEG and long-term prognosis. Afebrile seizures subsequently developed in 48.5% of patients with febrile convulsions in the 1st yr. From the analysis of factors associated with these 4 groups, group A was characterized by cryptogenic etiology, boy preponderance, positive family history, late onset after 6 mo. of age and very good prognosis. Group D was characterized by symptomatic etiology, a lower incidence of family history, a higher incidence of early onset within the first 6 mo. and poor prognosis for mental and physical development and seizures. Group B had features on A and D. Group C showed features different from group A and might have something in common with group D with very good prognosis.
