Ruptured pseudoaneurysm after ligation of patent ductus arteriosus--report of a successfully treated case

A 17-year-old man complicated with a rupture of pseudoaneurysm which was thought to be caused by ligation of patent ductus arteriosus, was successfully treated with emergency operation. The pseudoaneurysm had been present without showing any clinical symptom for 11 years. In hospital for operative treatment, the patient suddenly had a massive hemoptysis, followed by dyspnea and shock. In emergency operation the ruptured pseudoaneurysm and upper lobe of left lung were removed in an enbloc under partial extracorporeal circulation using femoro-femoral bypass. Postoperative course was uneventful, and the patient is up and well now 3 months after operation. Rupture of pseudoaneurysm after PDA ligation is difficult to be managed and has a poor prognosis. The case reported here is the first one with good success in Japan. It is thought to be contributed to good result that prompt treatment for massive hemoptysis was available because the patient was in hospital, and that proper extracorporeal circulation was used for control of peroperative bleeding.
