Characteristics of ventricular volume after Jatene procedure for complete transposition of the great arteries

From Aug. 1982-Dec. 1983, Lecompte's modification of Jatene procedure was employed in 10 patients with complete transposition of the great arteries (TGA) and 8 patients survived. Ventricular function and volume were studied in 7 survivors 1 mo. after correction, by means of cineangiography. The ages at operation ranged from 4 mo.-6 yr, and 6 patients were operated upon under 24 mo. of age. Patients (2) had simple TGA associated with pulmonary hypertension and complex TGA with large ventricular septal defect was found in 5 patients. Of 7 patients, 3 had undergone pulmonary artery banding previously. In all, anatomical correction was carried out under moderate hypothermia and extracorporeal circulation. Aortic cross-clamping was accompished and myocardial preservation was established with Young's solution, glucose-insulin-potassium solution and topical cooling. Right ventricular volume was calculated by Simpson's rule and left ventricular volume by area-length method. Using these volumes, ejection fraction was calculated. LVEDV was 0.57 .+-. 0.12 preoperatively and remained unchanged (57 .+-. 0.18). These results demonstrated that systemic ventricular function after Jatene procedure seemed to be better than that after Mustard or Senning procedure. The fate of arterial and coronary anastomosis and semilunal valve remained unknown in late stage.
