Hepatopathy in transplant patients acquiring hepatitis B virus after transplantation

Patients (408) submitted to renal transplantation from Jan. 1971 to Dec. 1983 at Hospital das Clinicas, University of Sao Paulo Medical School, Brazil, were analyzed. From this group, 41 patients (10%) contracted HBsAg (hepatitis B surface antigen), being previously negative in pretransplant test. From these, 24 (42%) became positive in < 12 mo. posttranplantation, 17 (42%) in the first 6 mo.; 8 patients (19.5%) became HBsAg negative during the transplantation evolution (5 in < 6 mo. and 3 in > 24 mo.). Among these 41 patients, 26 (63.4%) showed a high level of enzyme posttransplantation: glutamic oxaloacetic transaminase 162.9 IU/L (26-1670), glutamic pyravic transaminase 213.8 IU/L (28-1240) and .alpha.-glutamyl transpeptidase 149.2 IU/L (62-491) on the average. Ten patients died (24.4%). Seven died from infection, 1 from an hepatic insufficiency (micronodular cirrhosis) and 2 from other causes. The evolution of this hepatopathy is good in this experience.
