Prevention of peritonitis in continuous ambulatory peritoneal dialysis. Comparative study of 2 different protocols of training and bag changing

Patients (45) in end-stage renal disease were put on CAPD for a 3.5 yr peiod; the first 27 were trained following a nonrigid protocol; the remaining 18, and 13 patients of the aforementioned group, were trained or retrained, respectively, according to a much more rigid protocol, which included the use of a hand-disinfector (iodinated ethanol tincture). Overall incidence of peritonitis was 1 episode in 4 and 15, 1 patient-mo. for the 1st and 2nd group respectively. The probability of developing peritonitis in the first 6 mo. of treatment was 78% for the 1st group and 41% for the 2nd; the probability of developing 2 episodes during the 1st yr was half in the 2nd group as compared to the 1st. An only case of Staphylococcus epidermidis perotonitis could be registered in ptients trained following the 2nd protocol; this microorganism was implicated in 36% of peritonitis in the 1st group. It is relevant the preponderance acquired by S. aureus in the 2nd phase, when hand contaminaton was severely reduced.
