Three monoclonal rheumatoid factors (RF) (2 IgA .cntdot. .gamma. and 1 IgM .cntdot. .kappa.) were found in patients with Sjogren's syndrome and 1 monoclonal RF (IgM .cntdot. .kappa.) was found in Waldenstrom's macroglobulinemia. In the hemagglutination test, anti-idiotypic rabbit antisera against 3 M proteins inhibited the RF activity, indicating that the idiotypic determinants of monoclonal RF are closely related to the antigen-binding site. However, the idiotypic specificity of each of the 3 monoclonal RF was different. Idiotypic cross-specificity was examined between each of the 3 monoclonal RF and 60 nonmonoclonal RF obtained from patients with collagen or other diseases. The hemagglutination inhibition test demonstrated idiotypic cross-specificity between 1 monoclonal IgA RF and 4 nonmonoclonal RF and between 1 monoclonal IgM RF and 4 nonmonoclonal RF. The heterogeneity of some nonmonoclonal RF is apparently restricted.
