Intraoperative renal function and urinary beta 2-microglobulin and enzymes

Urinary .beta.2-microglobulin and enzyme were studied to estimate renal tubular function during elective operations in 31 patients. Five patients showed abnormal high values of .beta.2-microglobulin over 1400 .mu.g/l in urine. Their mean age was 74.8 .+-. 9.8. Higher mean age (P < 0.01) was accompanied with higher urinary pH (P < 0.01) and higher mean hourly excretion rate of urinary leucine aminopeptidase (P < 0.01). Their mean creatinine clearance tended to be lower (P < 0.2) but there was no significant difference. Geriatric patients seemed to have severe renal dysfunction during operation in view of urinary .beta.2-microglobulin, although their creatinine clearances were slightly depressed. The urinary enzyme increased in the patients with malignant disease and after furosemide administration. The patient with non-oliguric renal failure showed a marked decrease of urinary .beta.2-microglobulin concentration in spite of increase of urinary enzyme concentration at oliguric phase. Renal tubular dysfunction is important in the anesthetic management of geriatric patients.
