Provocation test by occupational challenge for hypersensitivity pneumonitis (farmer's lung)

Challenge exposures were carried out in 3 patients' cowshed for 10-30 minutes. Vital signs, blood counts, blood chemistry, respiratory function and chest X-ray were monitored for 2-6 days after exposure. In case 1, leukocytosis, observed at 6 h after exposure, improved within 24 h. In case 2, no significant changes were observed. In case 3, leukocytosis was observed at 7 h after exposure and improved within 24 h. Granular shadows appeared on the X-ray in the late phase of the course. The response patterns suggest in vivo immunological processes. It is suggested that the provocation test may be of use in the estimation of extrinsic antigens, and to explain the importance of environmental factors in the daily life of patients.
