Neurological disturbances in idiopathic hypereosinophilic syndrome

A 32 yr old female with bronchial asthma presented with pain, weakness and paresthesias of her hands and feet. Neurological examination revealed muscle atropy, brisk tendon reflexes and impairment of sensitivity to pain and temperature. An accelerated sedimentation rate as well as eosinophilia ranging from 9-40% were found. Abdominal pain, fever and acrocyanosis developed. Biopsy of 2-3 cm lesions on the feet led to a diagnosis of vasculitis. Increases on eosinophils and granulocytes in different stages of maturation were found in the bone marrow. Idiopathic hypereosinophilic syndrome was diagnosed in the presence of idiopathic increase in eosinophils and multisystem involvement. Neurological involvement is found in idiopathic hypereosinophilic syndrome but the association with myeloneuropathy is rare.
