The study refers to 200 patients with ischemic cerebrovascular accidents in the internal carotid artery distribution, in whom 204 operations of cerebral revascularization of the type of extra-intracranial anastomosis were performed between the superficial temporal artery (STA) and a cortical branch of the middle cerebral artery (MCA). Patients' mean age was 47 yr (the youngest, 7 the oldest, 78 yr). The most frequent lesion was thrombosis of the internal carotid (116 patients) and of the middle cerebral artery (46 patients). Anastomosis was performed, on the average, 9 wk after onset of the symptoms (at least 7 days and 4 yr afterwards at the most); 4 (2%) patients died, 9 (4.5%) improved slightly, 141 (70.5%) improved markedly and 46 (23%) recovered completely. The control group consisted of 50 patients in whom conservative treatment was used. Of this group 6 (12%) died, 16 (32%) either showed no imporvement or improved slightly, 20 (40%) improved markedly and 8 (16%) recovered completely.
