When hip function is severely impaired by disease or trauma, total hip replacement is the preferred treatment of choice. Total hip replacement has been carried out successfully over the last 3 decades and considerable improvements in design and surgical techniques have been made. Failures such as fracture of the prosthesis, stem under constant bending load, metallosis due to abrasionparticles and unfavorable design have increasingly been avoided and the most frequent failure of the prosthesis is the loosening of the stem. The numerous reasons for this have not yet been analyzed in detail. An analysis of the axial and radial stresses in the bone cement and cancellous bone of the femur in relation to such different parameters as cement or cancellous bone thickness, lip angle of the prosthesis, Youngs-Modulus of the prosthesis, or the length of the prosthesis was made.
