A case in a 53-yr-old man is reported. Swelling of the right ankle occurred in 1958 at the site of a splinter injury sustained in Jamaica in 1955. The swelling was excised. In 1968 he presented with painful swelling over the outer side of the right ankle. X-ray revealed no osteomyelitis. The swelling was excised. Phialophora [Exophiala] jeanselmei was isolated from specimens. The swelling recurred in 1980 following another accident. The patient was successfully treated with intravenous miconazole for 2 days followed by long term ketoconazole therapy at 400 mg/d.
