Myocardial infarction in southern Thailand

Among 444 cardiology inpatients seen in Hat Yai Municipal Hospital in 1981 and 1982, there were 103 patients admitted with myocardial infarction (MI) or its sequelae. Of these, 76 were acute MI; the remainder had heart failure or angina but their cardiogram showed pathological Q wave. Among these 103 patients, the risk factors were: 14% for diabetes mellitus; 26% with systemic hypertension; 24% of 62 patients had serum cholesterol greater than 250 mg/100 ml; and 18% of 27 patients had serum triglyceride greater than 150 mg/100 ml. Among the acute MI, there were 20% whose cardiogram showed evidences of previous transmural infarctions. Of the 103 patients, 21 died during their hospital stay. From the 53 patients in the 1981 series, there were a further 6 who died within 1 yr after discharge. Despite the possible underestimation of the incidence of MI, they represent 24% of the cardiology inpatients and about 0.2% of the total 24,940 hospital admissions. In terms of mortality, this represents 17% of the cardiovascular deaths, 4.8% of total medical death and 2.1% of total hospital deaths, figures which do not support the report on mortality of myocardial infarction in Thailand from the WHO statistics.
