A clinicopathologic study on orbital and ocular adnexal lymphoid neoplasms

Clinicopathologic features of lymphoid neoplasms of the orbit and ocular adnexa were documented in 63 cases treated at the Ophthalmic Division, Nagasaki University Hospital during the period from 1961-1981. There were 6 cases with malignant lymphoma, 29 cases with reactive lymphoid hyperplasia and 28 cases with inflammatory pseudotomur. According to the new histological classification proposed by the Japanese Lymphoma Study Group for non-Hodgkin's malignant lymphoma, malignant lymphoma was subclassified into 4 groups: 3 cases of small cell type, 1 case of medium-sized cell type, 1 case of large cell type and 1 case of pleomorphic type. The reactive lymphoid hyperplasia were further divided into 2 groups, those with and without recurrence. Each category as analyzed by histometrical data were grounded on Giemsa-stained sections that allowed a precise cytological diagnosis according to the rules of hematology. The ages of patients at the time of initial biopsy were frequently > the 4th decade, and the mean age was .apprx. 50, in each category. The clinical records and data of the present cases did not always correspond with the pathologic classification of the neoplasms, but the possibilities of pathological malignancy were suggested when variegated clinical symptoms were presented as compared with the length of clinical course. The possibilities of pathological malignancy were suggested when numbers of mitoses per 10 fields with .times. 400 magnification (20134 .mu.m2/field) on the Giemsa-stained sections were > 10 counts. The possibilities of pathological benignancy were suggested when percentages of plasma cells per all lymphoid cells were > 2%. The mean of shape factor was shown to be decreased, and its standard deviation was shown to be increased, in proportion to the increase of pathological malignant potentiality.
