Usefulness of transcutaneously monitored oxygen and carbon dioxide tensions during lung lavage and the relationship with simultaneously measured arterial blood-gas levels

During lung lavage for alveolar proteinosis, blood-gas levels in the patient change rapidly and extensively following the instillation and drainage of large volume of lavage fluid. In such a case, it is not easy to pursue the rapid changes by ordinary intermittent blood-gas analysis. Continuous transcutaneous O2 and CO2 monitoring was conducted throughout the lavage procedure, and the obtained data were compared with corresponding arterial blood-gas values. Ptc CO2 also rises with augmentation of PaCO2. The correlation of PtcO2 to PaO2 and PtcCO2 to PaCO2 was linear, respectively. The PtcO2 and PtcCO2 values responded quickly to changes in pulmonary vascular shunting due to the lavage manipulation. Transcutaneous O2 and CO2 monitor is easy and sensitive enough as checking blood-gas levels during lung lavage.
