Evaluation of cesium-131 myocardial scintiphotography in the detection of myocardial infarction

Cases (77) were studied with 131Cs myocardial perfusion scintiphotography. The quality and resolution of the .gamma. camera images were greatly improved when these images were managed by the PDP 8/M computer and image enhancement system (IES). It is a valuable technological improvement for the detection of myocardial infarction with 131Cs. Patients (77) were divided into 3 groups according to the clinical and laboratory examinations including ECG. Of 77 cases, 27 were normal control groups, all of them were found to be normal myocardial IES images. A total of 27 of the 77 cases were diagnosed as definite old myocardial infarction clinically. The location and size of myocardial infarction of 25/27 cases (92.5%) were clearly demonstrated by the IES images, while only 21/27 cases (77.7%) showed the abnormal images with radionuclide scanning. Of 77 cases, 39 were suspected old myocardial infarction, among them 1639 cases (41.3%) revealed positive IES images. The localization of infarction sites of the inferior and posterior wall was improved by IES images. Eleven of twelve cases (91.6%) showed infarct lesions in IES images, while only 58.3% (7/12 cases) showed infarct lesions in the myocardial scan. At the same time, IES imaging seems to be more sensitive than the routine ECG for the detection of myocardial infarction in the inferior and basal wall of the left ventricle. As a complementary diagnostic procedure, 131Ca myocardial scintiphotography is valuable for the detection of myocardial infarction.
