This article discusses why the DOTS (directly observed therapy, short-course) paradigm should be adapted for non-communicable diseases, and, with the "DOTS five-point policy package" as a template, shows how this could be implemented and rolled out in resource-poor countries, with special reference to sub-Saharan Africa.
