Kidney involvement in seronegative spondarthritides

Patients (50) of seronegative spondarthritides (SSA) having HLA B27 serotype but with negative rheumatoid factor were studied to find out the frequency, nature and degree of renal involvement. The patients were categorized into the following groups: Ankylosing spondylitis (23 patients); Reiter's disease (16); SSA-indeterminate (10); and SSA-rheumatoid arthritis (1). Nine patients had significant persistent proteinuria; renal biopsy in 7 of these patients showed proliferative glomerulonephritis (PGN). Immunofluorescent studies showed immune-complex deposits in only 2 of the 6 patients with PGN. The serum complement was normal and anti-nuclear factor was negative in all patients. Circulating immune-complexes were detected in 5 patients having proteinuria. Proteinuria and PGN occur in some patients of SSA.
