Diagnostic difficulties of bone lesions in neurofibromatosis

Of 15 cases with bone lesions due to neurofibromatosis, the etiology was correctly diagnosed prior to hospitalization in only three. Atypical findings included destruction of the calcaneus, a large cystic lesion of the ilium and a large tumor of the chest wall. Typical skin lesions were not obvious in these cases and had to be looked for. Skeletal survey helped in the diagnosis, but in 2 cases this was reached only after biopsy. Neurofibromatosis should be considered in every case of atypical skeletal lesion not corresponding to any other syndrome. A knowledge of the possible bony manifestations of neurofibromatosis is essential in order to avoid unnecessary operations.
