Therapy consisting of oral administration of 200-600 mg/day ketoconazole was given to 16 patients with Paracoccidioides brasiliensis infection, for periods ranging from 60 days to 19 months. Follow-up evaluation after drug withdrawal was performed for periods as long as 12 months. All patients had remission of symptoms and virtually complete regression of tegumentary and adenopathic lesions, with evident improvement of pulmonary involvement. According to established criteria 11 patients were considered as cured and 5 markedly improved. Side effects were slight; 2 patients presented transient serum transaminases increases.
