Reflux esophagitis in duodenal ulcer and therapeutic problems

A total of 147 patients with duodenal ulcers in the stage of exacerbation with concomitant reflux esophagitis were examined in order to reveal some mechanisms by which reflux esophagitis develops. Using esophagogastroduodenoscopy 3 degrees of esophageal mucosal inflammation were distinguished, showing a direct correlation of inflammation to the intensity of inflammatory-edematous deformation of the pylorus and duodenal bulb. The degree of reflux esophagitis is primarily determined by the length of the esophageal mucosa in contact with the gastric contents, based on data obtained by intraesophageal pH-metry. The efficacy of combined antiulcerous therapy including the local treatment of esophagitis and promoting the setting of disease remission over a relatively short period of time is emphasized.
