The relationship between the degree of portal hypertension measured by the gradient between wedged and free hepatic venous pressures, the endoscopic size of esophageal varices, the extrahepatic uptake of 99mTc phytate and the risk of gastrointestinal bleeding in a series of 53 unselected patients with chirrhosis, was studied. There was a significant relationship between size of esophageal varices and portohepatic gradient, and between extrahepatic uptake and portohepatic gradient. The sizes of the esophageal varices and portohepatic gradient were significantly higher in patients with gastrointestinal bleeding (n = 39) than in other patients (n = 14). The value of extrahepatic uptake is discussed.
