Oral Fe remains the safest and most economical route of administration. When the selected circumstances arise in which the parenteral route is better, i.v. Fe-dextran seems to be the most logical choice. The i.v. route is less protracted, much less painful, especially in patients with little muscle mass, and no more toxic. More physicians should be alerted to the efficacy of this route and consider total dose i.v. in preference to i.m. replacement.
