The widely-used Naclerio-Langer surgical method for treatment of giant lung bulla was occasionally associated with dangers of continuous air leak and development of new bullae, because of the weakening of the remaining lung and manifestation of postoperative obstructive lung disease. This method also seemed inadequate for treatment of elderly patients. The revised and improved operative method described in this study produced favorable results in aged patients. Complications decreased in frequency and the remaining lung was able to reexpand sufficiently.
