Superior limbal keratoconjunctivitis associated with thyrotoxicosis

A case of superior limbic keratoconjunctivitis is reported in a woman aged 57 yr with photophobia, epiphora and eye pain. Objective findings were bilateral superior limbic changes with conjunctival and ciliary congestion, edema and punctate rose-bengal staining. Several filiforme filaments were seen on the cornea. No protrusion of the eyes was seen. Treatment with topical antibiotics and steroids had no effect. Artificial tears (Lacril), combined with 0.67% silver nitrate solution had little effect. After treatment with 10% acetylcysteine (mucomyst) for a few days, the symptoms regressed and, after treatment for 6 wk, the symptoms disappeared. No congestion or edema of the conjuctiva was found, but only slight staining with rose-bengal. Four months after the initial eye symptoms, the patient presented thyrotoxic symptoms. The triiodothyronine and tyroxine levels in the blood were elevated. TSH levels were normal.
