Good patient acceptance and significantly decreased financial costs with the repair of groin hernias under local anesthesia on the ambulatory patient, are demonstrated. Management techniques that work smoothly for the patient and the surgeon are also discussed. This approach increases reliability of repair with comparable or increased safety and comparable or decreased complications. In this era of spiraling health care costs and increased patient awareness of health care options, the local anesthesia outpatient approach to hernial repair should be given serious consideration by all surgeons.
