A case with an esophagopleural fistula, an unusual complication of the ruptured esophageal diverticulum, is reported. A 42-yr-old man was referred with a diagnosis of the empyema of the left thorax. Chest X-ray showed complete collapse of the left lung and accumulation of fluid and free gas in the left thorax. On barium swallow there was a diverticulum at the lower part of the esophagus and from the top of this diverticulum, there was leakage of barium to the left thoracic cavity. After 2 mo. of tube drainage surgical resection of the fistulous tract and decortication of the left lung were performed. Postoperative course was uneventful except for serum hepatitis. The patient is now 6 mo. postoperative and engaging in full-time work. There are only a few reports about esophagopleural fistula secondary to the ruptured esophageal diverticulum and repaired by surgical intervention.
