Ocular positions following surgery of detached retinal

Ocular positions were examined 3 mo. to 8 yr and 3 mo. after surgery for detached retina. Unilateral retinal detachment in 34 patients was treated with diathermy, scleral buckling with silicone sponge explant and drainage of subretinal fluid. All patients had visual acuity of more than 0.4 after operation. Abnormal ocular positions were noted in 18 of the 34 cases (53%); this was more frequent in patients whose rectus muscles had been detached temporarily during operation. Abnormal ocular positions were less frequent in patients treated with placement of the explant at a distance from the rectus muscles. When scleral buckling was done leaving the end of the explant in contact with the rectus muscle, abnormal ocular positions were frequent; cases in which the explant was placed beneath the rectus muscle rarely represented abnormal ocular positions. When diathermy was done under or around the rectus muscles, abnormal ocular positions were frequent; however, diathermy itself caused abnormal ocular positions less often than temporary detachment of the rectus muscle or contact of the end of the explant with the rectus muscle. Abnormal ocular positions after retinal detachment surgery may be due to adhesion of the rectus muscle to the surrounding tissues based on interactions of surgical techniques and hemorrhages during operation.
