Crohn's disease of the upper gastrointestinal tract is rare, notwithstanding the fact that in recent years it was more frequently described. There still are controversies in the literature about the possibility of diagnosing it in the absence of intestinal involvement. A patient with gastric, esophageal and probably tonsillary Crohn's disease involvement, is described. The diagnosis was performed on surgical histological specimens. The absence of intestinal involvement was confirmed at necropsy. Main clinical, roentgenographic and endoscopic features are described. The disease was characterized by a particularly severe, catabolic pattern. Surgical, medical and nutritional therapy only gave a mild and transient benefit.
