Scintigraphy of the biliary ducts with 99mTc-diethyl-IDA in the differential diagnosis of jaundice

The material is based upon 242 patients, comprising 7 infants, 129 women and 106 men. All of these were submitted to biliary scintigraphy with 99mTc-diethyl-IM (acetanilidoiminodiacetate) on account of jaundice. In 172 cases of extrahepatic obstruction and 70 cases of hepatocellular disease, the diagnoses could be verified. In both groups of patients, it was found that if the indications for this investigation are limited to include patients with serum bilirubin values between 40-250 .mu.mol/l, significantly greater nosographic sensitivity could be obtained than in the total material. Under these circumstances, the nosographic sensitivity and specificity in patients with extrahepatic obstruction would be 0.74 and 0.85, respectively. The corresponding values in the group with hepatocellular disease were 0.68 and 0.85, respectively. The nosographic frequencies compared with the corresponding frequencies in the employment of other noninvasive methods are so low that the investigation will only obtain limited value in the differential diagnostic method in jaundice.
