A case of acute intramedullary spinal abscess is reported. Culture was negative and paralysis of 1 of the patient's legs and bladder occurred. The patient was treated operatively but without full remission of the neurological symptoms. The importance of early diagnosis and treatment in this potentially incapacitating condition is stressed.
