Mediastinal pseudocysts. A rare complication of pancreatitis

A 28 yr old male was admitted because of dyspnea on exertion, cough, hemoptysis and thoracoabdominal pain. The diagnosis of mediastinal pancreatic pseudocysts was suggested by the findings on chest X-ray examination and echocardiography, combined with a history of recurrent acute pancreatitis. Laparotomy was undertaken on account of suddenly occurring acute abdominal symptoms before a final diagnosis could be established by abdominal ultrasonic examination or computer tomography. During the operation, an anastomosis between the jejunum and a large pseudocyst was successfully established. One year after operation, the patient remained symptom-free and X-ray of the thorax showed normal conditions. The clinical picture in this rare condition may be alarming. The prognosis in mediastinal pseudocysts apparently does not differ significantly from the prognosis in cases with pancreatic pseudocysts limited to the abdominal cavity.
