A 45-yr-old man developed massive left-side pleural effusion. A right-side supraclavicular lymph node biopsy showed granulomatous reaction compatible with sarcoidosis. The PPD skin test was negative. The pleural effusion revealed mainly mesothelial cells and lymphocytes. Cultures of the pleural fluid produced no growth of tubercle bacilli and fungi. Just 1 mo. after administration of prednisolone, the widespread bilateral nodular shadows and the pleural effusion in the chest radiograph disappeared on the chest X-ray film.
