Four patients are presented, each showing the classic features of the anterior spinal artery syndrome: paraplegia of sudden onset and loss of pain and temperature perception, with preservation of touch, vibration and position senses. A short anatomical review of the spinal circulation is given and the etiology and differential diagnosis of this syndrome are discussed. The patients showed early improvement in muscle strength with varying degrees of sensory recovery, although some neurological deficits persisted in all cases.
