Caloric and rotational stimulation methods for the detection of vestibular asymmetry are presented. Warm and cold water caloric tests were correlated with post-rotative nystagmus. Detailed differential diagnosis and peculiarities of vestibular asymmetry in peripheral and hemispheric subcortical lesions are also provided. Hemispheric vestibular asymmetries are most often and well-defined in subcortical foci of temporo-parieto-frontal localization in the phase of decompensation. Experimental nystagmus toward the focus is strengthened. Hemispheric asymmetries of experimental nystagmus are greatly variable. The prevalent direction of nystagmus often varies not only during the disease but also in different vestibular stimulation. Hemispheric asymmetries are revealed only in a certain decompensation phase of the central subcortical vestibular mechanisms. Vestibular hemispheric asymmetries are replaced by bilateral nystagmus hyperreflexia as influences on the trunk increase while in the compensation phase of disease by symmetrically inhibited reactions.
