Conservation and treatment of the rectum in surgery of chronic ulcerative colitis (ulcerous rectocolitis and granulomatous colitis)

The importance of the safeguard of the rectum in the surgery of chronic inflammatory conditions of the colon is stressed. Once surgery has been decided upon, the problem lies in the choice of the type and technique of intervention. The value of intensive therapy (including total parenteral nutrition) of the underlying disease with the object of assuring the best possible conditions for the patient prior to surgery, is underscored. Equally important is topical treatment of the rectum in order to make it possible for the surgeon to carry out 1-stage or 2-ileorectal anastomosis. With this surgical technique, it is possible given appropriate local and general conditions, to avoid the trauma of definitive ileostomy which is often difficult to bear when taking into account the very young age of these patients. The experience with 86 patients undergoing colectomy, the immediate and long-term results, regarding complications, endoscopic findings, general conditions and quality of life, are reported.
