Extra-neural metastasis of pineal germinoma promoted by shunting device--a case report and review of the literature

A 19-yr-old boy was seen in Oct. 1975 with an 8 mo. history of progressive headache, nausea and vomiting. Neuroradiological examination suggested a pineal tumor and partial removal of the tumor with ventriculoperitoneal shunt was performed. Postoperative radiation therapy was applied to the whole head for the total doses of 5000 rad. Clinical symptoms were satisfactorily improved and he was discharged in Jan. 1976. In Sept. 1979, 4 yr after the initial treatment, he had an appendectomy and then egg sized tumors in the peritoneum were discovered. Histological diagnosis of the tumor was germinoma, identical to the pineal tumor. He was readmitted and abdominal irradiation was performed. Examination of the CNS failed to show any residual tumor. In March 1980 he was discharged and is in good condition, 5 yr after the initial onset of the symptoms and 1 yr after abdominal irradiation.
