Basic principles of intensive treatment of status asthmaticus

Three stages of status asthmaticus are distinguished depending on the character of pulmonary gas exchange disturbances. Stage I is characterized by hypoxemia without hypercapnia; in stage II hypoxemia and hypercapnia are observed stage III is hypercapnic coma. Principles of therapy of status asthmaticus are considered as based on the treatment of 158 patients. The therapy should be aimed a correcting the gas exchange disturbances, eliminating bronchial obstruction, restoring the sensitivity of bronchial adrenoreceptors and normalizing the body's internal medium. Infusion therapy, use of bronchial antispasmodics and corticosteroids, long-term peridural blockade, artificial pulmonary ventilation and other treatment methods are discussed.
