Left ventricular function in ostium secundum type atrial septal defect

Left ventricular function was studied by routine cardiac catheterization and biplane cine-left ventriculography in 43 preoperative and 21 postoperative cases with ostium secundum type atrial septal defect. Left ventricular end-diastolic volume index (LVEDVI), stroke index (SI), ejection fraction (EF), left ventricular stroke work index (LVSWI) and contractility index (Cont. I.) were calculated and compared with 26 normal subjects. In the small shunt group (Qp/Qs < 2) there were no significant differences in each indicies compared with the normal group. In the large shunt (Qp/Qs .gtoreq. 2) and younger (age < 30 yr) group, SI was significantly reduced but returned to normal range postoperatively. In the large shunt and elder (age .gtoreq. 30 yr) group LVEDVI, SI and LVSWI were significantly reduced, and SI and LVSWI remained within abnormally low range postoperatively. In the elder group there was a significant negative correlation between Qp/Qs and LVEDVI, Qp/Qs and SI, and Qp/Qs and LVSWI respectively. In large shunt group there was a significant negative correlation between age and LVSWI. In the postoperative group there was a significant negative correlation between age and SI, and age and LVSWI, respectively. In both the younger and elder postoperative groups there was a significant increase in LVEDVI compared to preoperative value and simultaneously significant decrease in EF and Cont. I. Differences between preoperative and postoperative LVEDVI correlated well with differences between preoperative and postoperative EF and Cont. I., respectively.
