Bilateral total knee and hip arthroplasty for rheumatoid arthritis in an adolescent

A 16-yr-old girl, severely crippled by Still's disease, underwent bilateral total knee and hip arthroplasty. Indications for this extensive procedure were painful, severely involved joints which incapacitated her. Because of severe temporomandibular and cervical spine involvement, anesthesia was performed with Ketamine, adjuvants and nasopharyngeal airway only. Although flexion contractures of both joints were entirely corrected intraoperatively, they recurred to some extent. One year and a half after surgery she was free of pain, relatively independent and had returned to school.
