The value of determination of sinus node recovery time in the diagnosis of sick sinus syndrome

The determination of sinus node recovery time (SNRT) following rapid atrial pacing in 7 patients with sick sinus syndrome (SSS) as well as 9 control subjects with no evidence of sinus dysfunction is reported. The SNRT of the 9 control subjects was 980 .+-. 180 ms, SNRT was 145 .+-. 87.5 ms, which were all within normal range. In 6 of the 7 SSS patients, the SNRT and SNRTc were above the normal limit, and only 1 case where the SNRT and SNRTc were normal, but sino-atrial block was found during rapid atrial pacing. All of the 7 cases showed the existence of sinus node dysfunction. The determination of SNRT following rapid atrial pacing provides definite clinical value in the diagnosis of SSS. SNRT > 1450 ms and SNRTc > 400 ms may reveal dysfunction of the sino-atrial node. Five cases (4 cases with SSS) exhibited 2.degree. or 1.degree. atrioventricular block during rapid atrial pacing at 90-125/min, suggesting the presence of latent damage of the atrioventricular node.
