Clinical, histomorphological, histochemical and therapeutic aspects of parapsoriasis

Patients (145) with different forms of parapsoriasis (92 with parapsoriasis [p.] nodularis, 38 p. guttata, and 15 p. varioliformis). Among these were 94 men (64.9%) and 45 (35.1%) women. Of the patients with p. nodularis, 75% were aged between 40 and 70 yr. The average duration of p. nodularis disease is 7-8 yr, p. guttata from 3 mo. to 5 yr. In 9 patients with p. varioliformis the duration of the disease did not exceed 3 mo. and in 6 patients .apprx. 2 yr, i.e., the disease was chronic. Clinically, p. varioliformis was characterized by the acute inflammatory nature of the eruptions with a trend to necrotization and marked polymorphism of the eruption elements (simultaneous occurrence of papules, pustules, vesicles, hemorrhages and necrotic crusts). Histomorphological, histochemical and enzymological examinations of the patients with p. nodularis and guttata revealed disorders in keratinization, some increase of the glycolytic process in the epidermis without any significant disturbance of the protein-synthesizing function of epidermal cells. Moderately marked inflammatory phenomena with a low level of metabolism of the infiltrate cells were found in the derma. Patients with p. varioliformis (5 cases) presented degenerative changes in the epidermis, formation of intraepidermal vesicles and significant disorders in the protein-synthesizing function of the epidermal cells. In the derma, vascular alterations were moderate and could not be interpreted as manifestations of allergic vasculitis. Clinico-morphological examinations suggest relationships between individual varieties of parapsoriasis. Complamin with methionine are recommended for therapy of parapsoriasis.
