A case of metastatic adenoid cystic carcinoma in left cerebellar hemisphere

A case of metastatic adenoid cystic carcinoma in the left cerebellar hemisphere is reported. A 45 yr old female had noticed a tumor in the right mandibular region 6 yr ago, and the tumor was totally extirpated with adjacent lymph nodes. Histologically, the tumor was verified as typical adenoid cystic carcinoma. She began to complain of headache and nausea 4 yr later, which gradually increased in severity and frequency. Later that year, she was admitted with cerebellar fits. The CT scan revealed a low density area, suggesting a cystic tumor in the left cerebellar hemisphere; neurologically definite cerebellar signs were also demonstrated on this side. Under the suspicion of astrocytoma or the metastatic tumor from right mandible, surgery was performed and the tumor was successfully removed. Histologically, the tumor was identical to that of the mandibular tumor. One year later, she was readmitted with gait disturbance and anorexia. Brain CT showed a increased low density area, suggesting the recurrence of the tumor. The cystic tumor was surgically open and an Ommaya tube was inserted in its cavity. Chemotherapy and immunotherapy were administered. The incidence of metastatic adenoid cystic carcinoma in cerebellar hemisphere is rare. The literature was reviewed.
